Once you change your name by Deed Poll, you need to use the executed Deed Poll to change all your documents into your new name.
 
 

Change Name on Passport

If you change your name, other than through marriage, or you are known by a name that is different from the one on your birth certificate, you almost invariably need a Deed Poll, before the passport office will issue you a new passport on your name. In these circumstances you need a Deed Poll, as proof of your name change, and you should send this with your passport application form to the Passport Office.  You will not be issued with a new passport without one.
 
If you already have a passport and you need to change your passport name, that is the name on your passport into your new name, you will need to use a deed poll to change your name.  In this case the passport office will re-issue a passport in your new name.  You will have to send the executed Deed Poll with your passport application form to get your name changed.
 
 
If you do not have any documentary evidence of your former name change.  It may be you changed your name or your parents changed your name, but no documentary evidence exists, or can be found.  In this case simply send one of our completed Deed Polls to the passport office with your application form and it will act as documentary evidence of your change of name.  The Deed Poll will act as documentary evidence of your name change from your birth name, the one on your birth certificate, to the one you have chosen to use for yourself.  Remember, when you apply for a deed poll you should use your birth name, the one on your birth certificate, as your current name and the name you want the passport issued in as your new name.
 
 
When you change your name by Deed Poll, the Deed Poll acts as documentary evidence of your name change.  Using our Deed Poll, you can change your passport name, what this means in practice is that a new passport will be issued in your new name.  It is important to remember than the passport office insist on original documents being sent to them, so you will need to send the original executed Deed Poll as proof of your name change to the passport office. The passport office will then issue a passport in this new name and return the original documents back to you.

Passport name change due to marriage

If you change your name due to marriage, for example, the wife takes her husbands surname, then the marriage certificate acts as documentary evidence of the name change.  Some people take on a double-barrel surname on marriage, using both partners surnames.  The Passport Office will accept the marriage certificate as evidence of the name change.  In this case the Deed Poll is not required, however, if the name is changed after the marriage and this change is not documented in the marriage certificate, then you will need a Deed Poll to change your passport name.
 
One thing to remember is that although the passport office may accept the marriage certificate as evidence of name change to a double-barrelled surname, most financial institutions do not.  In this case you will still need a Deed Poll as evidence of your names change.
 
 

Passport name change following a separation

Many people wish to revert to their maiden name, that is their surname before marriage or civil partnership.  To change your surname to your maiden name without a decree absolute, you will need to change your name by Deed Poll.  You can then apply for a new passport enclosing your Deed Poll document as documentary evidence of your name change.  This will allow you to change your passport name, by having a new passport issued in your chosen surname, usually the maiden name.
 
 

Passport name change for your child

If you want to change the name on your child's passport to a name that is different to the one on their birth certificate you will need a Child Deed Poll, unless the child is legally adopted.  In case of legal adoption, the legal adoption papers act as documentary evidence for the name change.
 
A parent or an adult with parental responsibilities over the child, such as a legal guardian, can apply for their child's Deed Poll.  You will need a Child Deed Poll to change the name of anyone under the age of 16.  You will need to send this Deed Poll to the passport office to have the name of the child changed on the passport or to have a first passport issued in the child's name.  We as a courtesy also provide, free of change, consent forms that you will need to do this.
 
If the child's name is different to the one on their birth certificate, the Passport Office will not issue the child passport without a Deed Poll as documentary evidence of the name change. Change driving license name

 
To change your name on a driving license requires a Deed Poll to show documentary evidence of your name change, pretty much like applying for a passport name change at the passport office.  You will need to get a Deed Poll in your new name and send this to the DVLA to get the driving license name changed into your new name.
 
 
To recap, when you apply for a change of name, except in limited circumstance such as when you get married and have a marraige certificate to prove the name change, you will need a Deed Poll.  You simply send the executed Deed Poll document with your other passport application papers, if you are applying for a passport in your new name.  The passport office will re-issue you a new passport in your new name.  If you apply to change name on a driving license, into your new name, then you need to follow the same process with the DVLA.
